Participants who register online and pay with a credit card or PayPal will be guaranteed placement at their requested location (*subject to availability and…) once payment is processed.


Participants who choose the deposit payment method (*if available);down payment plus selected payment plan, sent by email upon submission of online registration form, will be guaranteed placement upon receipt of payment and accompanying registration documents.


Registration documents and program fee must be received immediately upon registration or within 30 days of your submission of the Dskate Player Application Form based on your selected payment plan available on www.dskateminnesota.com. For payment arrangements, please contact a member of the Dskate team.

Upon receipt of registration documents and payment, additional information will be provided to you via email. The registration fees must be paid in full  in order to participate. 

Program fees include lunch, snacks, as well as on and off-ice activities for the duration of the Program (activities and included items vary per program selected).

Refund & Cancellation Policy

No refunds will be given except in the event of an accident or injury prior to the camp.

 

A doctor’s note will be required in order to receive a refund in the form of a credit note with regards to the cancellation. In that case, the participant will be issued a credit note which may be used for future programs offered by Dskate, less a 20% administration fee.


If the participant withdraws for any reason within 60 days of the Program, 50% will be refunded in the form of a credit note which may be used for future programs offered by Dskate.

Once the Program has commenced, there will be no deductions or partial refunds due to late arrivals or early departures.

If there is a medical reason as established by a medical professional at or outside the program, during program hours that necessitate a participant must leave early, a pro-rated refund credit note will be given.


Refunds will not be issued due to unforeseen events that will affect the execution of the program, namely closing of the facility due to damage to it due to superior force (weather, global pandemic, infrastructure problems of the facility not caused by Dskate Hockey Inc., etc...).


In the rare eventuality that any one of these situations may occur, Dskate Hockey Inc. will issue a credit note which may be used for future programs offered by Dskate Hockey Inc.

Terms & Conditions

It is the responsibility of the parent or guardian to drop-off and pick-up participants at the time determined in the schedule provided to them by Dskate Hockey Inc..


Dskate Hockey Inc. is not responsible for participants who are left unattended beyond the hours listed in the camp schedule.


The camp does not provide any equipment other than a team sweater and socks (unless otherwise stated in the specific program).

It is the participants’ responsibility to ensure they have equipment that is certified, approved and in good condition.


Dskate Hockey Inc. does not accept responsibility for misplaced, lost or stolen equipment or personal belongings during the camp.


Please ensure that ALL equipment and personal belongings, including clothing, diabetes supplies and any food items are clearly marked with the participant’s name, using permanent ink.

Miscellaneous

Registration Criteria

Dskate 2024 applications are open to all boys and girls of legal residence within Canada, the U.S.A. or the European Union, between the ages of 8 – 21 years old (age range may vary per program);


Up to 150 applicants may be selected based on the selection criteria stated above;


IF NOT ENOUGH APPLICANTS APPLY AND SELECTED FROM THE REGIONS OUTLINED ABOVE THE APPLICANTS WILL BE SELECTED BASED ON REGISTRATION ORDER ONLY.

 

To secure and confirm your space, payment and receipt of completed registration forms is required;


SELECTED APPLICANTS MUST CONFIRM ATTENDANCE UPON SUBMISSION OF COMPLETE REGISTRATION FORMS/PAYMENT.


Applicants must have Type 1 diabetes, currently take insulin to manage their diabetes, and be in the care of a health care professional who has cleared them for participation in this event (*unless otherwise noted);


Youth applicants with diabetes, regardless of their insulin treatment regimen (insulin shots, insulin pumps, or other) are eligible to apply;
